In the registration period 1 July 2007-30 June 2009
a total of 50 points minimum is required to qualify for a
Certificate of Currency, with points to be gained in at least two
categories.
Orthoptists who have graduated in this biennium
are eligible for a Certificate of Currency as your degree in
Orthoptics is proof of up-to-date professional training. Please
submit a Claim Form stating the name of your qualification and the
date your degree was conferred.
A Certificate of Currency lets the public and professional
bodies know that you have undertaken activities to further your
education relevant to orthoptics.
CPE CLAIM FORM
You may only apply for a Certificate of Currency with
renewal of your AOB Registration by completing a CPE
Claim Form. As each activity is undertaken list the information
on the form under the following headings:
- Activity Date/s:
- Name and Description of Activity
such as: conference title, web page address, journal/title/page,
name of clinic/hospital name and location,
- Accreditation Ref.
Code:
Write in the reference number from the accredited
activity reference list or "S/A" for Self Assessed
activities; and
- Points per Category.
- Total the columns
Remember - activities will only be
accepted if they have been accredited within 6 months of the activity
taking place and prior to 30 June 2009. Activities will not be
considered if submitted for accreditation with your CPE Claim.
Retain the completed CPE Claim
Form until your registration renewal is due,
then sign the declaration and enclose it with the biennial AOB registration
renewal (due from 1 July 2009).
DO NOT send proof of activity (eg receipt,
attendance certificate, reference pages, web page prints) but keep
these documents on file in case you are selected for random audit.
The only time a CPE Claim Form will be accepted
and a Certificate of Currency issued is at the time of AOB registration
renewal.
